BIRTH CERTIFICATE
$10 per certified copy
The Falmouth Town Clerk issues certified copies of the records of the birth of those people born in Falmouth and for those whose parents lived in Falmouth at the time of the child’s birth. Birth certificates are restricted records if the child is born out of wedlock. These records are limited to the Mother, the Father (if listed on the birth record) and the child (16 years or older) with proper I.D.

DEATH CERTIFICATE
$10 per certified copy
The Falmouth Town Clerk issues certified copies of the records of death only for persons who died in Falmouth or whose residence was listed as Falmouth on the death certificate if they died in Massachusetts.

MARRIAGE CERTIFICATE
$10 per certified copy
The Falmouth Town Clerk issues certified copies of the Marriage Certificates of those people who filed their Marriage Intentions in Falmouth. You may have been married in Falmouth but your marriage will be recorded in the town where you filed their Marriage Intentions.

Marriage certificates are restricted records if the Bride's or Groom's parents were not married at the time of the Bride's or Groom's birth. These records are limited to only the Bride or Groom with a proper I.D.

DOG LICENCE
Neutered or Spayed 8.00 per year
Non-Neutered or Non-Spayed 15.00 per year
Puppy (under 6 Months old) 5.00 first year
A dog license is required for each dog you own or keep that is six months or older. In the Town of Falmouth a dog license is issued in relation to the expiration of the dog's rabies vaccination. A rabies certificate, which includes current rabies tag number and expiration date, is required.

If your dog is lost you can be contacted quickly. Both the Dog Officer and the police have current dog lists, which help them contact owners in the event of a problem. The license also shows that the dog has had it rabies shot. This is for the benefit of the dog officer when he is handling a lost or injured dog.
Business Certificates "DBA"

Any person, whether individually, a partnership, or a corporation conducting business under any name other than the complete name of the person or corporation conducting the business, must file with the Town Clerk’s Office a Business Certificate (also referred to as a "DBA" certificate). (M.G.L. c.110, s.5)

A business certificate does not give you permission to operate a business, it only registers the name. It is your responsibility to obtain all the necessary licenses and/or permits required by the town.

The fee for a Business Certificate is $40 and is good for four years.

If you are paying online you will need to have your signature notarized before uploading the Application. If you will be paying in person the Clerk’s Office will notarize it.
Yard Sale Permit

Unless specific written authorization is given by the Board of Selectmen, not more than two (2) such sales shall be held on any one (1) residential premises in each calendar year nor more than one (I) such sale in any period of three (3) consecutive months, and any such sale shall not continue for more than two (2) consecutive days.

Any sign advertising such sale shall not exceed two (2) square feet, and there shall be no off- premises signs allowed.

Any person violating any of the provisions of this chapter shall be punished by a fine not exceeding fifty dollars ($50.00) for each offense. Each day that a violation hereto exists shall be deemed a separate offense.
